Hi,
as you mentioned I would visit Athens and Istanbul during another trip. So let's start in Bremen :)
From Bremen you can easily get a train to Berlin. You have to change trains in Hamburg. So you can either add a here or just pass through. Krakow - Auschwitz (Oswiecim): Get a local train from Krakow to Auschwitz, which runs nearly every hour (7:14, 9:14,10:16,11:17...)

So to get to Dubrovnik, you have to add some more stops :) As the connection Budapest - Sarajevo is canceled, you have to go via Zagreb: Budapest - Zagreb - Sarajevo - Mostar - Ploce:

You can then use a bus to Split. From Split you can enter a train again :)
The next train station to the Plitvice is Vrhovi-Plitvi Jezera: So you can use one of the night trains, which goes from Split to Zagreb and exit there in the middle of the night :D maybe you'll find some other connections on the website of the railway company of Croatia. I think it's a taut program :D even if you plan to travel one month :) Maybe you cut off Mostar and Dubrovnik, too.

Have you been looking into flying to/from an airport somwhere in the Balkans? This would make it quite easy doing this trip using a 10in22 IR plus a few regular (bus) tickets for the remainder.
I was thinking of (with a 10in22):
1 Bremen - Berlin
2 Berlin - Prague
3 Prague - Krakow
4 Krakow - Budapest
5 Budapest - Wien
6 Wien - Salzburg
7 Salzburg - Hallstatt - Salzburg
8 Salzburg - München
9 München - Füssen/Neuschwanstein - München
10 München - Bled/Ljubljana
To get to Oswiecim, use regular tickets for a day trip from Krakow or take the night train from Prague and get off early at Oswiecim, continue to Krakow in the evening.
From Bled/Ljubljana to Zagreb get a regular ticket at the station; Zagreb to Plitvice Lakes and onwards to Dubrovnik and Mostar by bus.
This would only leave you with finding a convenient airport in the area for the flight back home.
To compare whether or not the rail pass pays off you can compare prices on the websites of the different railway companies:

Overall I think the 10in22 will be more convenient and better value as well...26,50€ per trip is quite good value. Of course there will be offers that are cheaper on some parts of the route (for instance on Budapest - Wien there are quite cheap advance fares available) however these are 1) subject to availability 2) not always available online and 3) limit your flexibility during the trip. Also, there are some parts where even cheap advance fare wont save you money or is not available (online) at all, for instance on Krakow - Budapest.
Regarding night trains: On the suggested route you would be able to use night trains from Prague to Oswiecim/Krakow; Krakow to Budapest; München to Bled/Ljubljana.
On all these trains reservation is compulsory, so you would have to spend at least a few €€ for a seat (no seats available on Krakow - Budapest btw).
Green square means no extra fees, yes. Hopping on trains will work out though on most day trains of your route, only trains to/from Poland would require a reservation (but I'd recommend using night trains on these routes as mentioned above).

If you click on the connection, you'll see it's a bus connection :) Autobusni Kolodvor. It's based on the link I've posted.

Several connections daily - as usual you just turn up at the bus terminal and buy your ticket there. Journey time is 3:30 to 4 hours. Price ~ EUR 16.
