Probably because there are no trains, the international line out of Thessaloniki has been closed for around 3 years.

Fly - or use the ferry from Greece to Italy and then train.

Hello.

1)
From Thessaloniki, take a bus to Igoumenitsa. Read the information here:

There are at least three buses a day. The journey takes about four hours. Tickets cost around 40 EUR.

2)
Take a ferry from Igoumenitsa to Italy.
For example to Ancona. Other options include Bari or Venice.
To find out how to get from the bus station to the ferry port, click here. Also how to get to the train station in Ancona (local bus or on foot).
And of course links to book your ferry ticket. The ferry trip is overnight. Tickets start from 70 EUR. The ferry from Igoumenitsa to Bari is cheaper. But from Bari to Milan it is a longer journey by train. Your choice :)

In Ancona change to the train which will take you to Milan in 3h30.

The ticket price depends on your train connection and is around 30 EUR.
