Are the Milan S lines covered with the Interrail pass or do I have to pay something extra and need a special ticket for them ? How much is the ticket for the Milan s-lines ?

depends-this is one of the little mysteries. As they are run by both the state-trains=FS=trenitalia and the local regional FNM, LeNORD, which does not honour IR. it does not matter what it says on the trains-but who owns the tracks. the PASSANTE is FS, but the connection into that other station (Cavorna?not sure) is not.
Anyway-IN the city of MIlano you better had buy a dayticket @3€, which also covers all trams and the metro IN the city. There are complicated zones to travel out of it.
it is also unclear for the trains that now run from Centrale into the airport at Malpensa-the last part of this is owned by leNORD and hence IR should not be valid.
But I just read that in this region (LOmbardia) FS-locals and the leNORD are about to merge. No idea what happens after that.

Yup, LeNord and Trenitalia will merge...curious too what happens than with InterRail validity on former FNM/LeNord lines. :)
