No, not all trains have them. Their number is increasing though. :|

But are they exactly as you would find in a house or are they like airplanes where you need to buy an adapter?

Yeah, you won't need an adapter. :) It's the standard european plug.

Probably not an easy question to answer but is there any geographical factors eg german trains have them but others don't.... Just wondering if there's any logic to it in order to plan having sufficient phone battery etc

No. There are no factors ... some have, some not... in Western Europe you will find power plugs more often in trains than in Eastern Europe.
But I never ran out of energy. Just open your eyes. :o
Ask the staff in the trains. Fast food restaurants also have them - just check the corners. And of course in hostels you will always find them.

In case any of you are travelling to Ireland here are details of sockets on our trains
Q. Is there sockets/plugs onboard IÉ intercity services?
A.
Yes on the following routes
All Dublin/Cork direct trains (MK IV fleet ~ click here to see an image) there is two 220V sockets in each standard class coach behind seats 05 & 06 and in Premier class there is a 220V socket for each seat 01-44.
On all of the new trains (silver trains ~ 22000 Class fleet click here to see an image) currently being introduced into service on all routes. There is a 220V socket in every seat Premier class and all standard class coaches.

Note that we don't use the European plug so you will need a travel adaptor as we use the three pin socket
