any ideas how easy this is to do? Looking at the rail maps it seems not, anyone taken a similar journey by coach or such other method?

By train you can go via Serbia and Macedonia. From Bar there is a daily sleeper car to Skopje and from there you can get trains to Thessaloniki. Otherwise you could bus accross Albania (time consuming, but would be an awesome experience)
