Definitely a nice trip; personally I think september is a nice time for such a trip, did a four weeks trip once in september in central and eastern europe and had very nice weather, campsites and cities themself werent as crowded as in summer.
From Milano to Prague there are many possibilities: You could travel through Austria (Graz, Vienna, Salzburg and other nice cities; good connections to Prague) or through Switzerland and then Austria or Germany.
I dont know if you prefer visiting cities or if you also would like to make a hiking trip or relax at a nice lake? Two things you could do in Austria for instance. ;)

So the weather is okay in september?
We would love to stop at a lovely lake, would be great to have a destination where we can relax! Do you have any suggenstions? But Austria, isnt that a bit boring? ;)

Thanks, SiDUDe; no worries, bohl90. 8)
I would recommend going to Carinthia in this case, lots of nice lakes where you definitely can have a good time. Easily reachable from Milano with changing trains in Venezia.

:D
Of course you could also go via Switzerland I guess Zurich might also be a nice place to party - although maybe not quite als relaxing as being in Austria.

There are several direct connections with Thalys, but you will have to pay a supplement of 33€. Travel time with Thalys is 4h09.
However there are alternatives route that aren't as expensive as Thalys but they are slower and require changing trains several times. Calculate about eight hours for the journey.
Detailed information https://rail.cc/en/interrail-train-reservation/thalys/41 here.
